Residency Verification

Dear Parent,

We hope that you and your families continue to be healthy and safe.  When we left campus in March, we were optimistic that we would be together on campus at the beginning of the year.  While this ended up not being the case, we are still excited to welcome our students back to school on August 31^st, in a Distance Learning Model.  In the coming weeks, we will share information about materials/device distribution, Virtual Curriculum Nights and more.

Many of you have asked about Residency Verification, which would typically happen in May, and due to the pandemic was postponed.  At this time, we are now asking families to complete residency verification. There are two options to verify residency this school year.  Please review the two options below and decide which one works best for you:

 

Please Note:  If your child is an incoming Kindergarten student or just registered as a new student for the 2020-21 school year, you will not need to verify residency.

 

Online:

1. Complete the fillable PDF of the Verification of Residency Document and sign.  Verification of Residency Form or Second Family Residency Form  It must be your signature – you cannot type your name.  You may scan and upload the document with your signature.  If you have more than one child, you must complete a document for each child.
2. Scan or upload a picture of a document on the approved list.  The list can be found on the Verification of Residency Document.  The bill/document must be dated after July 1, 2020.

3. Email both the signed Verification of Residency Document and the approved bill/document to muraokaschool@cvesd.org with your child’s name in the subject line.

In Person Drop-Box Option:

1. Come to Muraoka and take or complete the form at our station behind the gate, next to our main office (see photo). Bring and attach a copy of a document on the approved list and drop off in our locked box.
2. Our drop box will only be available to drop off during the hours of 6:30 am – 4:00 pm to ensure privacy of documents.

In Person Option:

1. Come to Muraoka on Friday, August 14^th between the hours of 8:00 am-3:45 pm or on Tuesday, August 18^th between the hours of 1:00 pm 6:00 pm.  Please wear a mask and adhere to all social/physical distancing protocols. At this time, we can assist with making copies or answering questions regarding Verification of Residency. This time is STRICTLY reserved for Verification of Residency.
2. Pick up and complete the Verification of Residency Document and sign.  If you have more than one child, a document needs to be completed for each child.
3. Bring a document or copy of a document on the approved list.  The list can be found on the Verification of Residency Document.  The bill/document must be dated after July 1, 2020.

 

Verification of Residency Documents Accepted:

• Mortgage book or statement
• Homeowner’s association billing statement
• New rental contract/lease, or current payment receipt with landlord contact info
• Letter on apartment complex or mobile home park letterhead, signed by the landlord, stating that parent/guardian/caregiver/ lives there
• Bill: Gas & Electric, Water, Sewer, Trash, Cable, Landline Phone, Pay Stub, Voter Registration, Property Tax Payment Receipt
• Correspondence from a government agency

Estimado Padre,

Esperamos que usted y sus familias continúen sanos y seguros.  Cuando salimos del campus en marzo, éramos optimistas de que estaríamos juntos en la escuela a principios del nuevo año escolar.  Aunque esto no fue el caso, todavía estamos emocionados de dar la bienvenida a nuestros estudiantes para el nuevo ano escolar que tendrá inicio el 31^deagosto, en un Modelo de Aprendizaje a Distancia.  En las próximas semanas, compartiremos información sobre materiales/distribución de dispositivos, Noches de Currículo Virtual y más.

Muchos de ustedes han preguntado acerca de la Verificación de Residencia, que normalmente ocurriría en mayo y debido a la pandemia, tuvo que ser pospuesta.  En este momento, ahora estamos pidiendo a las familias que completen la verificación de residencia. Hay dos opciones para verificar la residencia este año escolar.  Revise las dos opciones y decida cuál funciona mejor para usted:

Nota: Si su hijo/a será un estudiante de Kínder o simplemente se registra como un nuevo estudiante para el nuevo año escolar 2020-21, usted no tendrá que verificar la residencia.

 

En línea:

1. Complete el PDF rellenable del Documento de Verificación de Residencia y firme.  Debe ser su firma – no puede escribir su nombre.  Puede escanear y subir el documento con su firma.  Si tiene más de un hijo/a, debe completar un documento para cada niño.
2. Escanee o cargue una imagen de un documento de la lista aprobada.  La lista se puede encontrar en el Documento de Verificación de Residencia.   El recibo de utilidades /documento  debe tener la fecha después del 1 de julio de 2020.
3. Envíe por correo electrónico los dos documentos. El documento de verificación de residencia con su firma y el recibo de utilidades/documento aprobado a  muraokaschool@cvesd.org con el nombre de su hijo en el sujeto del correo electrónico.

 

Opción en persona con caja Drop-Box:

1. Pueden venir a la escuela Muraoka y tomar o completar el formulario en nuestra estación detrás del cancel, junto a nuestra oficina principal (ver foto). Traiga y adjunte una copia de un recibo de utilidades/documento de la lista aprobada y déjela en nuestra caja cerrada.
2. Nuestra caja para dejar documentos sólo será disponible durante las horas de 6:30  am  – 4:00  pm para garantizar la privacidad de los documentos.

Opción en persona:

1. Puede venir a la escuela Muraoka el viernes 14 de agosto entre  las  8:00 am-3:45 pm o el martes 18 de agosto entre el horario de  1:00 pm 6:00 pm.  Por favor, use una máscara y adhiera a todos los protocolos de distanciamiento social/físico. En este momento  podemos ayudar con la realización de copias o responder preguntas con respecto a la Verificación de Residencia. Este tiempo está ESTRICTAMENTE reservado para la Verificación de Residencia.
2. Recoja y complete el Documento de Verificación de Residencia y firme.  Si tiene más de un hijo, es necesario completar un documento para cada niño.
3. Traiga un documento o copia de un documento de la lista aprobada.  La lista se puede encontrar en el Documento de Verificación de Residencia.   El recibo de utilidades utilidades/documento  debe tener la fecha después del 1 de julio de 2020.

 

Verificación de los documentos de residencia aceptados:

• Libro o estado de cuenta de la hipoteca
• Estado de cuenta de la asociación de propietarios
• Nuevo contrato/arrendamiento de alquiler, o recibo de pago actual con información de contacto del arrendador
• Carta en el complejo de apartamentos o membrete del parque de casas móviles, firmada por el propietario, que indica que el padre/ tutor / guardián / vive allí
• Factura: Gas y Electricidad, Agua, Alcantarillado, Compañía de basura y reciclaje, Cable, Teléfono de línea en casa (no celular), Talón de Pago, Registro Electoral, Recibo de Pago de Impuestos de Propiedad
• Correspondencia de una agencia gubernamental
